Retatrutide: what the research shows

Retatrutide is an investigational peptide that acts on three hormone receptors at once: GIP, GLP-1 and glucagon. What it is, what the published trials measured, what is still unknown and where it stands with regulators in October 2026.

What retatrutide is

Retatrutide, development code LY3437943, is a single synthetic peptide with agonist activity at three receptors: the glucose-dependent insulinotropic polypeptide receptor (GIPR), the glucagon-like peptide-1 receptor (GLP-1R) and the glucagon receptor (GCGR). It was developed by Eli Lilly and Company. [1] [3]

In laboratory tests the molecule showed balanced activity at the glucagon and GLP-1 receptors and stronger activity at the GIP receptor. In obese mice it lowered body weight and improved glucose control; the authors attribute part of the weight effect to increased energy expenditure through the glucagon receptor, on top of the lower food intake driven by the GIP and GLP-1 receptors. [1]

How it has been studied

Retatrutide went from a first-in-human study to phase 2 trials in obesity and in type 2 diabetes, and then to the phase 3 TRIUMPH programme. The phase 2 and phase 3 trials below were randomised, double-blind and placebo-controlled, and all were funded by Eli Lilly. [1] [2] [3] [4]

The phase 2 diabetes trial also had an active comparator, dulaglutide, another GLP-1 receptor agonist: at two of the higher retatrutide doses, HbA1c fell more than with the comparator. [3]

TrialWho took partLengthMain published result
Phase 1, single ascending dose (2022)Early clinical study, after studies in miceSingle dose, followed up to day 43Safety and tolerability similar to other incretin-based molecules [1]
Phase 2, obesity (2023)338 adults with obesity or overweight48 weeksMean body-weight change of up to -24.2% at the highest dose, against -2.1% with placebo [2]
Phase 2, type 2 diabetes (2023)281 adults with type 2 diabetes36 weeksHbA1c down by up to 2.02 percentage points at 24 weeks, against 0.01 with placebo; body weight down by up to 16.9% at 36 weeks, against 3.0% [3]
Phase 3, TRIUMPH-1 (2026)2,339 adults with obesity, without diabetes80 weeksMean body-weight change of up to -25.0%, against -3.9% with placebo; knee-osteoarthritis pain and sleep-apnoea events were also lower in the subgroups studied [4] [5]

Safety findings reported so far

In the published trials the most common adverse events were gastrointestinal: nausea, diarrhoea, vomiting and constipation, mostly mild to moderate. In the phase 2 obesity trial they were dose-related and partly reduced by a lower starting dose. [2] [3] [4]

The phase 2 obesity trial also recorded dose-dependent increases in heart rate, which peaked at 24 weeks and declined afterwards. In the phase 2 diabetes trial there were no reports of severe hypoglycaemia and no deaths. [2] [3]

The longest published trial lasted 80 weeks; effects beyond that period have not been reported. [4]

Regulatory status (October 2026)

Retatrutide is not an approved medicine. The US Food and Drug Administration states that retatrutide is not a component of any FDA-approved drug, has not been found safe and effective for any condition, and cannot be used in compounding under federal law. [6]

The FDA has warned companies that sold semaglutide, tirzepatide or retatrutide directly to consumers for human use, with dosing instructions, while labelling the products ‘for research purposes’ or ‘not for human consumption’. Research material is not a medicine: it has not been reviewed for safety, effectiveness or quality in people and must not be used in people. We sell retatrutide only for in vitro laboratory research, without instructions for use. [6]

Storage and stability in the lab

Peptides can degrade through chemical routes such as oxidation, deamidation and hydrolysis and through physical routes such as aggregation, and they are generally more stable in the dry state than in solution. [7]

Lyophilised vials are therefore normally kept cold, dry and away from light until they are reconstituted. The storage statement on the product page and on the batch certificate takes precedence.

How to check a batch

A useful certificate of analysis names the testing laboratory, the batch number printed on the vial, the test date and the methods used. Identity is usually confirmed by mass spectrometry, where the measured mass should match the expected molecular mass, and purity is measured by HPLC.

Where the manufacturer supplies a batch report, we link it on the product page; products without one do not claim one.

Common questions

Is retatrutide approved?

No. As of October 2026 retatrutide is an investigational molecule: the FDA states that it is not a component of any approved drug and has not been found safe and effective for any condition.

How is retatrutide different from semaglutide and tirzepatide?

Semaglutide is a GLP-1 receptor agonist and tirzepatide a dual GIP and GLP-1 receptor agonist; retatrutide adds activity at the glucagon receptor. Our guides on semaglutide and tirzepatide cover those molecules.

What is the largest retatrutide trial published so far?

TRIUMPH-1, a phase 3 trial that randomised 2,339 adults with obesity and without diabetes for 80 weeks (ClinicalTrials.gov NCT05929066), published in 2026.

Can I buy retatrutide for personal use?

No. Everything we sell is for in vitro laboratory research by adults aged 21 or over. It is not for use in people or animals, and we give no advice on use or dosing.

This guide summarises published research and regulatory information. It is not medical advice. Everything we sell is for in vitro laboratory research only, by adults 21+, and not for use in people or animals.
